This is a fully remote position, open to applicants in California.
• Offer brokerage support to seniors and individuals with disabilities under contract with the California Department of Human Services.
• Conduct orientation and training on program regulations.
• Enroll participants along with their direct care workers.
• Assist participants in creating cash management and spending plans for their monthly budgets.
• Monitor the health and safety of participants.
• Maintain progress notes in compliance with NASW standards.
• Manage a caseload of up to 125 program participants.
• Ensure required contact with assigned participants, including monthly phone calls.
• Address participant inquiries and maintain email correspondence as necessary.
• Resolve issues faced by participants.
• Provide practical skills training in finding community resources; recruiting, selecting, hiring, training, and overseeing employees; purchasing goods and services; and managing monthly budgets.
• Assist participants with enrollment in the California Self-Determination Program and necessary paperwork.
• Help identify community resources, services, and supports to achieve participants' life objectives.
• Aid participants in developing spending plans and scheduling employees within their monthly budgets.
• Assist direct care workers with required employment documentation.
• Report and address participant complaints and grievances.
• Act as a mandated reporter for any allegations of abuse, neglect, or exploitation.
• A BA or BS degree in a related field is required, or at least 4 years of relevant experience as an alternative.
• A minimum of two (2) years of experience working with individuals with disabilities and/or senior citizens is required.
• Knowledge of Medicaid services is preferred.
• Bilingual proficiency in English and Spanish is preferred.
• Must reside in California.
• Ability to manage one's own schedule and work independently with minimal supervision.
• Strong skills in Microsoft Office applications.
