This is a fully remote position, open to applicants in California.
• Oversee, coordinate, and assist in the execution of Social and Community Services Programs.
• Develop a high-performing team.
• Lead a team and facilitate the removal of obstacles for Members.
• Aid in the creation of new workflows and job aids.
• Provide coaching and recommendations aimed at enhancing performance.
• Monitor and report on operational processes and productivity metrics.
• Prepare reports and documentation for leadership review.
• Attend and/or lead regular meetings.
• Adapt to a flexible schedule, including evenings and weekends as necessary.
• Integrate IEHP Quality Program objectives, including HEDIS, CAHPS, and NCQA Accreditation.
• Foster relationships with community-based organizations and leaders.
• Propel new initiatives and secure tangible business outcomes.
• Minimum of three (3) years of experience in one or more of the following areas: Supervision/Management, Managed Care/IPA/Clinic Setting, delivering presentations/trainings to diverse audiences.
• Experience with State Health Programs (Medi-Cal/Medicare), case coordination, Community Health Work/Promotora Programs, community outreach, Behavioral Health/Substance Use Disorder Program, case management, and acute care.
• Proven experience leading a team and managing operations with accountability for results.
• A Master’s degree in Social Work from an accredited institution is required.
• An active, unrestricted, and unencumbered Clinical Social Worker (LCSW) license issued by the California Board of Behavioral Sciences is mandatory.
• Comprehensive knowledge of Medicare, MediCal, HEDIS, MCAS, DHCS, and NCQA.
• Familiarity with the social determinants of health.
• Awareness of community resources available within the SB/RIV counties.
• Understanding of healthcare services and the role and necessary skill sets of Community Health Workers (CHWs).
• Sensitivity to multi-cultural communities and awareness of the implications of unchecked bias and judgment.
• Insight into services for Members facing homelessness, serious mental illness, substance use disorders, domestic violence, and acute medical health issues.
• Knowledge of whole person health and the impact of social deprivation.
• Bilingual proficiency in one of IEHP's threshold languages is highly desirable: English, Spanish, Chinese, or Vietnamese.
• Exceptional verbal, communication, and interpersonal skills.
• Relationship-building skills that include empathy, motivational interviewing, coaching, social support, and conflict resolution.
• Proficient in computer skills including typing, data entry, email, Word, Excel, Outlook, and PowerPoint.
• Capacity to handle multiple projects while adhering to ethical and legal guidelines, identifying mandatory reporting scenarios, and demonstrating strong planning, organization, time management, and critical thinking skills.
• Strong analytical abilities, quantitative goal-setting, data interpretation, leadership, agility, and collaboration skills.
• Proficient in evidence-based communication techniques such as Motivational Interviewing or similar empathy-based communication strategies.
• Ability to organize and prioritize tasks, work under pressure, solve problems, accurately recall information, maintain attention to detail, and work independently with minimal supervision.
• Capability to engage team members in various settings including by phone, at Members’ homes, in the community, and elsewhere.
• Ability to cultivate relationships with community-based organizations and leaders, including faith-based communities.
• Ability to forge effective relationships with team members while primarily working in the field.
