This is a fully remote position, open to applicants in California.
• Oversee, coordinate, and facilitate the execution of Social and Community Services Programs.
• Build and lead a high-performing team.
• Plan, assign, schedule, and assess team tasks while ensuring adherence to quality standards.
• Recruit, train, develop, conduct performance evaluations, and implement corrective actions when necessary.
• Establish professional objectives and address barriers to members’ care.
• Create processes and workflows that enhance the experiences of both members and team members.
• Ensure seamless transitions of care and promote member wellbeing.
• Educate, guide, and support the SCS team in collaboration with hospitals and providers.
• Discuss intricate cases, assist with escalated member needs, and suggest further actions.
• Develop meeting agendas and lead monthly team gatherings.
• Monitor strategic goals, outcomes, KPI data, productivity, and contributions to HEDIS compliance.
• Relay field intelligence to the SCS Manager.
• Address service concerns and represent Community Health and IEHP leadership.
• Collaborate with SCS teams, provider offices, and community organizations.
• Cultivate relationships with healthcare providers, community organizations, and stakeholder partners.
• Collaborate with providers, community organizations, and leaders to enhance quality metrics and achieve PHM Bold Goals 50x2025.
• Attend and represent the SCS department at IEHP meetings and external events.
• Coordinate special projects alongside the SCS Manager and community partners.
• Share best practices, enhance education and outreach, and promote care connections and disease prevention.
• Conduct chart audits, quality assurance checks, coaching, clinical support, and shadowing activities.
• Assist with the resolution of complex and escalated member cases.
• Facilitate regular case conferences.
• Identify performance challenges, care gaps, and opportunities for operational efficiency.
• Train team members on IEHP policies, regulations, procedures, workflows, and electronic medical management systems like MedHok.
• Monitor continuous quality improvement related to healthcare access, primary care, prevention, social determinants of health, and HEDIS gaps.
• Implement the LEAN model for program standards, processes, and training.
• Act on behalf of the Manager when required.
• Participate in meetings, provide training, and perform advanced representation as needed.
• Minimum of three (3) years of experience in supervision/management, managed care/IPA/clinic settings, or delivering presentations/trainings to diverse audiences.
• Experience with State Health Programs (Medi-Cal/Medicare), case coordination, community health work/Promotora programs, community outreach, behavioral health/substance use disorder programs, case management, or acute care.
• Proven track record in leading a team and managing operations with accountability for outcomes.
• A Master’s degree in Social Work from an accredited institution is required.
• An active, unrestricted, and unencumbered LCSW license issued by the California Board of Behavioral Sciences is mandatory.
• Familiarity with Medicare, Medi-Cal, HEDIS, MCAS, DHCS, and NCQA is required.
• Understanding of social determinants of health, community resources in San Bernardino/Riverside counties, healthcare services, CHW roles, and multicultural communities.
• Bilingual proficiency in one of IEHP's threshold languages is highly desirable: English, Spanish, Chinese, or Vietnamese.
• Exceptional verbal, communication, and interpersonal skills.
• Skills in motivational interviewing or similar empathy-based communication techniques.
• Proficient computer skills including typing, data entry, email, Word, Excel, Outlook, and PowerPoint.
• Strong planning, organization, time management, critical thinking, analytical, problem-solving, and collaboration skills.
• Ability to organize and prioritize workload, work under pressure, work independently, and maintain attention to detail.
• Capability to engage team members via phone, in members’ homes, in the community, and other environments.
• Ability to foster relationships with community-based organizations and leaders.
• Must be able to lift 25 lbs.
• 75% driving travel is required in assigned Riverside and San Bernardino County territories.
• Must be willing and able to travel to members’ homes.
