This is a fully remote position, open to applicants in Florida.
• Oversee a caseload of patients who are high-risk and socially complex.
• Perform psychosocial assessments in person and via telephone with patients and/or their caregivers.
• Examine psychological and social determinants that create barriers and develop interventions to address gaps in care and needs.
• Offer short-term counseling services when necessary.
• Facilitate coordination of services to ensure patients and families comprehend treatment plans.
• Collaborate with the care team to enhance quality and patient experience outcomes, including participation in High-Risk Huddles.
• Reach out to community-based services to assist patients in maintaining their independence at home.
• Establish and implement shared care goals with patients and their families.
• Cultivate relationships with community-based organizations and provide resources that promote patient independence.
• Collaborate with providers, support staff, care teams, patients, families, caregivers, facilities, third parties, external providers, and community resources.
• Master’s degree in social work.
• Must hold a valid license as an LCSW or LISW (based on state requirements).
• Required to maintain a valid driver’s license and a clean driving record.
• Ability to exercise sound clinical judgment and communicate effectively in both written and verbal forms.
• Above-average computer proficiency.
• Must be self-motivated and able to communicate effectively with professionals across various disciplines and programs.
• Capable of effective time management, balancing multiple tasks, working within tight deadlines, resolving issues, identifying patient service trends, determining system enhancements, and implementing changes.
• Ability to share knowledge with others and recognize the importance of fostering performance improvement while ensuring patient satisfaction and efficiency.
• Preferred experience in Care Management within a community-based setting or equivalent.
• Preferred experience working with chronic, complex, and/or behavioral health populations.
• Candidates may need to undergo a background check in the state of Florida.
