This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Oversee and directly supervise a designated team that assists members with intellectual, developmental, and TBI disabilities.

• Ensure that staff utilize Trillium Connections workflows to conduct thorough assessments and create care plans as the needs of members evolve.

• Foster a team-oriented, person-centered strategy to address members' physical health, behavioral health, and social determinants of health.

• Supervise teams to promote integrated care through collaboration among multidisciplinary teams, including members, guardians, primary care providers, behavioral health and IDD providers, specialists, nutritionists, and pharmacy.

• Implement evidence-based and industry-standard best practices to establish processes, procedures, and workflows for efficient care coordination and management.


⛳️ Requirements

• A Bachelor’s degree or licensure as an RN, along with five (5) years of experience in care management, case management, or care coordination for complex individuals with I/DD or TBI; OR a Master’s degree in a human services field or licensure as an RN, and three (3) years of such experience.

• A valid driver’s license is required.

• Candidates must reside in North Carolina and be located within 75 miles of Trillium’s South Central Region, which encompasses Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, and Wayne counties.

• Ability to travel within the catchment area as necessary.

• Previous experience focusing on the Innovations Waiver is preferred.

• A minimum of two (2) years of supervisory experience is preferred.

• All candidates are required to pass a drug test as a condition of employment.
