SUD Nurse Practitioner

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Perform telehealth evaluations to evaluate substance use, co-occurring disorders, and overall well-being.

• Diagnose health issues and develop personalized treatment plans.

• Prescribe medications, oversee side effects, and modify treatment as necessary.

• Collaborate with Care Navigators, Recovery Coaches, Case Managers, judicial partners, community organizations, the Holon care team, and external providers.

• Act as a clinical consultant for complex or high-need cases.

• Provide education to clients and their families regarding treatment plans, medications, and recovery support strategies.

• Ensure accurate and timely documentation in accordance with ethical, legal, and medical standards.

• Engage in team meetings, professional development, and ongoing training.

⛳️ Requirements

• Active and unrestricted Nurse Practitioner license in Wisconsin.

• Valid DEA license (and CDS if applicable).

• Several years of experience in behavioral health, addiction medicine, or psychiatry.

• Background in addiction medicine or psychiatry.

• Preferred board certification in specialty.

• Strong skills in documentation.

• Proficient in Electronic Health Record (EHR) systems.

• Compassionate attitude toward individuals facing substance use disorders and mental health challenges.

• Ability to collaborate effectively within integrated and team-based care environments.

• Clear and confident communication skills in a virtual setting.

• Organized, adaptable, and capable of navigating complex systems.
