Social Worker – LCSW

This is a fully remote position, open to applicants in California, +1 more state.

📋 Description

• Provide support for the behavioral health requirements of patients within an assigned caseload.

• Offer brief, evidence-based interventions for conditions such as depression, anxiety, grief, mood regulation, and substance use disorders.

• Collaborate with prescribing providers to assist in the management of psychotropic medications.

• Aid in ensuring treatment adherence while monitoring medication effectiveness and any potential adverse effects.

• Work alongside internal psychiatrists and clinical care teams to address urgent psychotropic concerns.

• Engage in caseload reviews and participate in clinical team discussions.

• Facilitate communication regarding patients' behavioral health requirements.

• Timely complete and document patient assessments.

• Adjust treatment plans based on measurement-based care approaches.

• Act as a subject matter expert in behavioral health, sharing insights and resources.

• Provide care that respects and honors the diverse values, beliefs, backgrounds, and social, cultural, and linguistic needs of patients.

• Accurately document patient interactions, referrals, and other relevant information in the electronic health record.

• Ensure that encounter notes are secured in compliance with company policy.

• Carry out additional responsibilities as assigned.

⛳️ Requirements

• A Master’s degree in Social Work is required.

• Current independent licensure in California or Washington is mandatory.

• A minimum of 2 years of prior experience in healthcare or behavioral health is essential.

• Strong organizational and problem-solving abilities are needed.

• Experience in geriatric care is highly preferred.

• Capability to manage complex patient needs and multiple clients with varying requirements.

• Effective written and verbal communication skills are necessary.

• Ability to handle multiple tasks simultaneously with minimal supervision and adhere to deadlines.

• Must thrive in a fast-paced, dynamic environment and collaborate effectively within a team.

• Proficient in Microsoft Office Suite (Outlook, Excel, PowerPoint, Word).

• Experience with Electronic Medical Records (EMR) is required.

• Reliable internet connectivity is essential.

• Knowledgeable in navigating internet-based tools and applications.

• Proficient in computer documentation practices.

• Exhibits a high level of professionalism.
