This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Collaborate with a registered nurse case manager in a joint process of assessment, planning, and advocacy.

• Address members’ benefit plan and/or health requirements.

• Investigate available options and resources for members facing financial, transportation, safety, and housing challenges.

• Conduct telephonic and electronic outreach to members throughout the entire lifecycle population.

• Encourage members to adopt positive lifestyle and medical decisions.

• Partner with clinical staff to aid members in accessing medical, financial, and behavioral health community resources.

• Work schedule is Monday to Friday from 8am to 4:30pm EST.

• No weekend, holiday, or travel coverage required.

⛳️ Requirements

• Residency in North Carolina is mandatory.

• A Master's Degree in Social Work is required.

• Must hold active, unrestricted licensure as one of the following: LSW (Licensed Social Worker), LMSW (Licensed Master Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker).

• A minimum of 2 years of experience in medical social work or case management is essential.

• Capability to acquire and maintain multi-state licensure (at least 10 states – willing to obtain additional licenses) before completing the new hire training.

• Must be available to work during EST hours.

• Availability Monday to Friday from 8am to 4:30pm EST is required.
