This is a fully remote position, open to applicants in Pennsylvania.
• Evaluate, identify, secure, and oversee services for program participants tailored to their specific needs.
• Address participant requirements through various communication channels.
• Adhere to compliance standards and deadlines for assessments, service plans, visits, calls, and incident reporting.
• Record each visit and communication in mandated systems in a comprehensive, person-centered approach.
• Offer training and support to participants and consumer employees involved in direct care services.
• Maintain and utilize internal databases to monitor participant information and interactions.
• Assist participants in acquiring Accessibility Adaptations, Specialized Medical Equipment, Non-Medical Transportation, and additional services.
• Exemplify company values in all interactions and responsibilities.
• Travel across Monroe and Pike counties and neighboring regions.
• Bachelor's degree is mandatory; degrees in social work, psychology, or related areas are preferred.
• A minimum of three years of experience in a social service or healthcare-related environment is required as a substitute for the bachelor's degree.
• Practicum experience is advantageous.
• Experience working with individuals with disabilities or elderly populations is preferred.
• At least one year of experience in LTSS, Service Coordination, or Case Management is preferred.
• Strong communication, customer service orientation, integrity, job knowledge, reliability, problem-solving abilities, interpersonal skills, productivity, quality, self-improvement, and teamwork are essential.
• Proficient in relevant computer systems and technology for the role.
• Capability to perform activities such as reaching, stretching, stooping, sitting, walking, and standing.
• Must be able to lift up to 20 pounds independently.
• Valid driver's license and reliable transportation are required.
• Must be willing to travel throughout Monroe and Pike counties and adjacent areas.
