This is a fully remote position, open to applicants in Illinois.
• Assess the needs and requirements of members to ensure they achieve or maintain optimal wellness.
• Direct members and their families towards suitable care and wellbeing resources.
• Facilitate interactions with payer sources, healthcare providers, interdisciplinary teams, and others involved in the care of members.
• Provide coverage for Care Coordinators 1 and 2 and document activities within the clinical system.
• Mentor, coach, precept, and support team members.
• Assist with training initiatives and the onboarding process for new hires.
• Offer guidance, support, and development of team processes.
• Foster a positive work environment.
• Provide field-work assistance and oversee team members during their field visits.
• Elevate issues to the manager and support team management as needed.
• Submit daily reports on pending cases and review compliance dashboards.
• Assist with internal and external audit initiatives.
• Complete field-work support and review associated reports.
• Coordinate services while monitoring and evaluating case management plans in relation to members' personal goals.
• Oversee patient care through assessments, data analysis, conversations, and active care planning.
• Ensure patients make progress toward desired outcomes.
• MSN, Illinois Registered Nurse (RN) with one year of experience in conducting comprehensive assessments and providing formal services for the elderly, or Licensed Clinical Professional Counselor (LCPC), or Licensed Clinical Social Worker (LCSW).
• Must be a resident of the State of Illinois.
• At least 2 years of experience in healthcare and/or case management.
• Minimum of 2 years of experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community Based Services and/or managed care organizations.
• Proficient in using electronic information applications/software programs, including electronic medical records.
• CCM certification is preferred.
• Experience in health promotion, coaching, and wellness is preferred.
• Knowledgeable in community health and social service agencies, along with additional community resources, is preferred.
• Must pass tuberculosis (TB) screening if selected.
• Valid state driver’s license is required.
• Maintain personal vehicle liability insurance that meets required limits.
• Home internet connection with a minimum of 25 Mbps download and 10 Mbps upload speed.
• A dedicated workspace that is free from ongoing interruptions to protect member PHI/HIPAA information.
• Willingness to travel within Illinois up to 25% of the time.
