RN - Registered Nurse Navigator, Triage - Population Health

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Deliver nursing triage services and health guidance via phone and, when appropriate, in-person.

• Evaluate patient acuity and identify the necessity for medical assistance.

• Provide diagnostic testing outcomes along with relevant education.

• Maintain accurate, relevant, and timely communication through telephone and electronic messaging.

• Offer clinical advice based on established protocols.

• Assess patients over the phone and document chief complaints, symptoms, medical history, pharmacy details, and contact information following set protocols.

• Initiate and coordinate emergency interventions when required.

• Prioritize urgent calls and relay essential information to triage providers.

• Educate patients and their families.

• Schedule or facilitate emergency consultations.

• Coordinate patient care and requests across various departments, specialties, and locations.

• Assist in the orientation and training of new staff members.

• Keep a record of ongoing patient and customer interactions and respond promptly to their needs.

• Reinforce provider instructions and ensure compliance with treatment plans.

• Clarify medical conditions to patients.

• Address patient concerns utilizing clinical protocols and service recovery strategies.

• Complete and direct patient documentation promptly.

• Propose process enhancements and solutions.

• Coordinate care and services with external organizations, including insurers, home health agencies, and medical facilities.

⛳️ Requirements

• Graduate from a Specialty Training Program in Nursing (Required).

• Licensed Registered Nurse in Pennsylvania.

• Basic Life Support Certification.

• A minimum of 5 years of nursing experience is preferred.

• Experience in ICU, ED, or Critical Care is highly preferred.

• Bachelor of Science in Nursing (BSN) is preferred.

• Specialty certification is preferred.

• Strong computer skills are required.

• Proficiency in Microsoft Word, Excel, PowerPoint, SharePoint, Windows, and Teams.

• Clinical skills are essential.

• Knowledge of the healthcare industry.

• Familiarity with medical devices and tools.

• Understanding of patient safety protocols.

• Availability to work rotating day and night shifts, including 7a-7:30p or 7p-7:30a.

• Participation in weekend and holiday rotations is required.

• Travel of less than 25% may be necessary for orientation/training, meetings, or other business requirements.
