This is a fully remote position, open to applicants in United States.
• Offer remote, patient-focused case management assistance and education to assigned patients
• Establish and nurture therapeutic relationships with patients assigned to you
• Provide patient-centered education on treatments, disease conditions, and prescribed medications
• Oversee a specific caseload through scheduled outreach, adherence monitoring, and precise database documentation
• Facilitate interactions between patients and both internal and external clinical partners
• Tailor interactions to meet the unique needs of individual patients, considering their health literacy and program specifications
• Engage in customer meetings, training sessions, and orientation programs
• Exhibit clinical proficiency and system knowledge through required evaluations
• Assist with special projects and additional responsibilities related to contract performance and business requirements
• Comply with contractual obligations, company policies, quality standards, and regulatory requirements
• Work hours are from 11:00 a.m. to 8:00 p.m. Eastern Time
• Valid, unrestricted Registered Nurse (RN) license in your primary state of residence
• Compact license is mandatory
• At least two (2) years of clinical experience as an RN
• Proficient in Microsoft Office applications and basic database navigation
• Excellent verbal and written communication abilities
• Capability to handle multiple priorities, meet deadlines, and work independently in a remote setting
• Access to cable broadband or fiber optic internet service with a minimum of 10Mbps download and 5Mbps upload speed
• Bachelor of Science in Nursing (BSN) or active enrollment in a BSN program is preferred
• Prior experience in case management or care coordination is preferred
• Familiarity with pharmacy operations, pharmaceuticals, life sciences, or healthcare business settings is preferred
