RN Case Manager III

This is a fully remote position, open to applicants in Arizona, +16 more states.

📋 Description

• Deliver member-focused, evidence-driven case management across various products.

• Assist members with significant needs in navigating the healthcare landscape.

• Ensure safe transitions between different care environments.

• Conduct personalized assessments and determine members' care objectives.

• Coordinate services across a range of care settings.

• Connect members with community resources that meet their needs.

• Alleviate the burden of illness for individuals and families.

• Contribute to the reduction of overall healthcare expenses.

• Communicate effectively with physicians and healthcare providers.

• Negotiate and secure necessary resources.

• Work efficiently towards departmental performance benchmarks.

• Perform additional duties as assigned.

⛳️ Requirements

• Associate's degree in Nursing or equivalent professional experience in a related field.

• At least 3 years of experience beyond the degree in clinical or acute care nursing.

• Current, unrestricted RN license in the state of residence.

• Preferred experience working with vulnerable and complex populations in clinical, home care, or telephonic settings.

• Strongly preferred direct case management experience.

• Experience with various age groups and diverse ethnic and socioeconomic backgrounds.

• General understanding of the healthcare continuum.

• Solid clinical knowledge and familiarity with chronic conditions, ADLs/IADLs, and social determinants of health.

• Experience in managing multiple computer systems and tools.

• Must be legally authorized to work in the United States at the time of application.
