RN, Case Manager – Float Team

This is a fully remote position, open to applicants in Florida.

📋 Description

• Evaluate and identify the needs of patients and families utilizing the nursing process.

• Collaborate with the Interdisciplinary Team (IDT) to create personalized Plans of Care.

• Deliver direct clinical, palliative, supportive, psychosocial, spiritual, cultural, physical, and biological care.

• Offer telephone reassurance and assess the necessity for in-person visits.

• Organize and document patient/family interactions in the EMR.

• Complete initial and semi-annual evaluations.

• Explain available services and address any patient/family inquiries or concerns.

• Initiate skilled nursing interventions, acquire physician orders, and finalize essential documentation.

• Represent the organization at designated facilities and facilitate referrals across various service lines.

• Communicate clinical information to the IDT by employing the SBAR method.

• Conduct home visits to evaluate safety, medication and nutritional adherence, DME compliance, and the ability to live safely within the community.

• Educate patients, families, caregivers, and healthcare professionals regarding disease progression, palliative interventions, caregiving, the dying process, and safety measures.

• Report changes in patient conditions to IDT members and other healthcare professionals.

• Assess hospice referrals and admissions in collaboration with the IDT.

• Participate in daily IDT collaboration meetings, presenting both oral and written reports.

• Supervise care provided by Community Health Workers and Home Health Aides upon request.

• Report to a company-designated location and may need to stay overnight during emergencies.

• Perform additional responsibilities as assigned.

⛳️ Requirements

• Valid RN license in the state of employment.

• At least one year of nursing experience is required.

• Hospice or hospital experience is preferred.

• Employees at PACE must undergo Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs.

• Previous experience with an EMR/EHR system is necessary.

• A valid driver's license and automobile insurance in accordance with Company policy.

• Reliable transportation to adhere to the visit schedule.

• Capability to operate equipment with visual and auditory components.

• Proficient communication skills in English, both verbal and written.

• Ability to conduct home visits for assessments.

• Capacity to perform essential job functions and meet physical requirements, including lifting, bending, pushing/pulling, and kneeling, with or without reasonable accommodation.

• Current BLS certification for healthcare professionals from the American Heart Association or Red Cross.

• Successful completion of competency requirements.

• Agreement to drug and/or alcohol testing following a conditional job offer.

• Ongoing adherence to the Drug-Free Workplace Policy.

• Eligibility verification through the Florida Care Provider Background Screening Clearinghouse for employees serving Florida affiliates.
