RN Care Manager

This is a fully remote position, open to applicants in Kentucky.

📋 Description

• Design, evaluate, and implement intricate care management strategies primarily for members with physical needs.

• Assess member requirements, obstacles to care access, and social determinants affecting health.

• Create continuous care plans while collaborating with healthcare providers, specialists, and community organizations.

• Recognize challenges and barriers to care and execute appropriate care management interventions.

• Coordinate efforts among members, families/caregivers, and care provider teams to guarantee person-centered care.

• Monitor and follow up on member status, changes in condition, and advancement towards care-plan objectives.

• Offer resource support for employment, housing, participant direction, independent living, justice, and foster-care services.

• Streamline care management and ensure timely access to essential services.

• Conduct outreach via telephonic, digital, home, or other site interactions.

• Gather, document, and uphold member information and care management activities to comply with regulatory standards.

• Educate members and families/caregivers regarding disease management, care gaps, provider instructions, care options, referrals, and healthcare benefits.

• Provide leadership insights on avenues for enhancing care quality and cost-efficiency.

• Adhere to all policies and standards while performing additional assigned tasks.

⛳️ Requirements

• A degree from an accredited nursing school or a Bachelor’s degree in Nursing.

• 2–4 years of relevant experience.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is required.

• Preference for candidates residing in Kentucky or the eastern or central time zones.

• Preference for applicants holding an active Kentucky State or Compact RN license.

• Preference for experience in medical-surgical or other inpatient units, clinical outpatient environments like home health, or care management.

• Availability Monday through Friday, from 8 AM to 5 PM CT or ET.
