Resource Consultant

This is a fully remote position, open to applicants in Virginia, +1 more state.

📋 Description

• Provide guidance, mentorship, education, and support to individuals, seniors, and individuals with disabilities utilizing Personal Options services.

• Conduct home visits with participants at least biannually.

• Exhibit proficiency in Personal Options services, relevant West Virginia waiver programs, and Palco policies.

• Inform and mentor participants or their designated representatives regarding program rights, responsibilities, and potential risks.

• Assess and monitor participants' capacity to engage in the program.

• Aid in the creation and execution of initial and revised service/spending plans.

• Oversee the utilization of service/spending plans.

• Address and report member or representative complaints and grievances.

• Serve as a mandatory reporter for suspected abuse, neglect, exploitation, and other necessary incidents.

• Employ a person-centered approach during interactions with participants.

• Accurately document participant visits on a company-provided laptop.

• Log events and generate reports electronically.

• Complete daily administrative duties, including visit scheduling and responding to company communications.

• Adhere to Palco's operational procedures and privacy policies.

• Engage in shadowing opportunities and mandatory training sessions.

• Timely distribute new information to assigned participants.

⛳️ Requirements

• A minimum of three years of experience working with the public.

• High School Diploma or equivalent is required.

• Preferred experience includes roles such as patient navigator, community connector, community health worker, lay health advisor, health educator, or case manager.

• Strong verbal and written communication skills.

• Capability to electronically document visits.

• Proficient in Microsoft Office, Microsoft Outlook, and Internet usage.

• Effective problem-solving abilities.

• Willingness to travel within a designated geographic area to meet participants in their homes.

• Open to working with individuals of all ages and disabilities.

• Possession of a valid driver's license and reliable transportation.

• Active prepaid or contract cell phone plan for ongoing communication.

• Ability to pass a criminal background check and maltreatment check.

• Travel is required.
