Resource Consultant

This is a fully remote position, open to applicants in Virginia, +1 more state.

📋 Description

• Provide guidance, mentorship, education, and assistance to individuals, senior participants, and those with disabilities using Personal Options services.

• Conduct home visits with participants at least once every six months.

• Exhibit proficiency in Personal Options services, West Virginia waiver programs, and Palco policies.

• Inform participants and their designated representatives about their rights, responsibilities, and potential risks associated with the program.

• Assess and monitor participants' ability to engage in the program effectively.

• Aid in the creation and revision of service and spending plans.

• Track the utilization of service and spending plans.

• Address complaints and grievances from members and their representatives.

• Serve as a mandatory reporter for any suspected abuse, neglect, exploitation, and other required incidents.

• Employ a person-centered approach during interactions with participants.

• Accurately record participant visits using a company-issued laptop.

• Log events and generate reports electronically.

• Execute daily administrative duties, including scheduling visits and managing emails and calls.

• Adhere to Palco operational procedures.

• Engage in shadowing and ongoing training sessions.

• Distribute new information to assigned participants as necessary.

• Protect company equipment and data in compliance with company policies and HIPAA privacy regulations.

⛳️ Requirements

• A minimum of three years of experience working with the public is essential.

• Preferred experience includes roles such as patient navigator, community connector, community health worker, lay health advisor, health educator, or case manager.

• A High School Diploma or equivalent is required.

• Strong verbal and written communication skills are necessary.

• Competence in electronically documenting visits is required.

• Proficient in Microsoft Office, Microsoft Outlook, and Internet usage.

• Effective problem-solving skills are important.

• Must be willing to travel within a designated geographical area to meet participants in their homes.

• Must have a willingness to work with individuals of all ages and disabilities.

• A valid driver’s license and reliable transportation are required.

• An active prepaid or contract cell phone plan is necessary for ongoing communication.

• Ability to pass a criminal background check and maltreatment check is mandatory.

• Travel is a requirement of the position.
