Rehabilitation Service Specialist – West

This is a fully remote position, open to applicants in Canada.

📋 Description

• Identify necessary rehabilitation assessments to evaluate Participants’ potential for improvement and define rehabilitation requirements.

• Organize rehabilitation assessments and services as needed.

• Ensure that services and plans are tailored to individual needs, focused on goals, compliant with program mandates, safe, cost-efficient, and of high quality.

• Create comprehensive Rehabilitation Plans in collaboration with Participants, their families/caregivers, Rehabilitation Service Professionals, and Veterans Affairs Canada Case Managers.

• Serve as the service integrator for medical, psycho-social, and vocational rehabilitation services.

• Empower Participants to find solutions by exploring community-based interventions and resources.

• Recognize barriers or risk factors that may lead to less than optimal outcomes.

• Actively develop and execute strategies to enhance outcomes.

• Deliver rehabilitation services to veterans of the Canadian Armed Forces and their families.

• Travel occasionally across Western Canada as necessary.

⛳️ Requirements

• A bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related discipline.

• A professional health designation or Certified Vocational Rehabilitation Professional (CVRP or CVRP-F).

• At least 3 years of experience in providing medical, psycho-social, or vocational rehabilitation services, or a combination of these.

• Experience working with individuals facing mental or physical health challenges that create life barriers.

• Employment is contingent upon obtaining federal security clearance – Reliability Status.

• Willingness to travel within the region as required.

• Bilingualism (French/English) is considered an asset.

• Experience collaborating with employers, community resources, and training institutions.

• Current and former members of the Canadian Armed Forces (CAF) are encouraged to submit their applications.
