Registered Nurse – Triage Nurse Specialist

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Conduct telephonic triage to assess the most suitable level of care.

• Provide requested assistance and/or virtual monitoring services.

• Deliver recommendations, education, and care guidance utilizing decision-making tools, clinical expertise, established workflows, protocols, and resources.

• Engage in care coordination efforts to minimize readmissions, improve chronic disease management, and oversee health risk and injury reporting.

• Arrange provider appointments and facilitate communication among providers.

• Guarantee precise and prompt documentation within the EMR, adhering to best practices, guidelines, and workflows.

• Take part in virtual monitoring, reporting, and escalation to designated support services.

• Assist in the business operations and continuity of Conduit Health Partners.

• Contribute to process improvement, professional growth, peer development, and peer review activities.

• Work evenings, weekends, and three holidays annually.

• Uphold performance standards with a focus on safety, quality, patient experience, and organizational strategic initiatives.

⛳️ Requirements

• RN license is mandatory in the relevant state(s).

• Multistate/Compact RN Licensure is preferred.

• ADN or Diploma in Nursing is required.

• BSN is preferred.

• A minimum of 1 year of acute care nursing experience is required.

• Triage experience is preferred.

• Familiarity with EPIC Electronic Health Record is preferred.

• Capability to provide care suitable for neonates, infants, pediatrics, adolescents, adults, and geriatrics.

• Minimum internet speeds of 100 Mbps download and 20 Mbps upload are required at primary and secondary work locations if working remotely.

• Ability to work shifts is necessary.

• Occasional travel by car to off-site locations is required, including in adverse weather conditions.

• Manual dexterity, finger dexterity, color vision, far and near acuity, and the ability to hear alarms, telephones, and audio recordings are necessary.
