This is a fully remote position, open to applicants in Colorado.
• Identify patients for proactive interventions utilizing screening criteria, reviewing medical records, applying payor models, calculating medical risk scores, or processing referrals.
• Evaluate patients’ medical, functional, and social conditions to create personalized care plans, recommendations, or referrals.
• Coordinate internal and external services to address social determinants of health and community care needs.
• Review and adjust patient care plans and interventions as necessary.
• Perform remote nursing assessments and acute symptom triage based on evidence-based protocols.
• Assist patients in self-management and behavior modification through health coaching, care navigation, care coordination, and educational support.
• Provide education to healthcare team members, patients, caregivers, and families regarding transitions, triage, conditions, and self-management strategies.
• Collaborate with physicians and multidisciplinary healthcare teams to ensure timely and high-quality care.
• Engage in rounds or case conferences and consult resources in social work, nutrition, pharmacy, rehabilitation, and behavioral health.
• Cultivate partnerships with hospital care management, post-acute providers, and other care managers.
• Take part in system and regional process-improvement initiatives.
• Conduct root cause analyses of extended post-acute stays, inappropriate utilization, and readmissions.
• Monitor key data elements and metrics while keeping abreast of industry, regulatory, technology, and market trends.
• Advocate for the mission, vision, and values of Intermountain Health.
• A current Registered Nurse (RN) license in the state of practice.
• A Bachelor of Science in Nursing (BSN) from an accredited institution; degree verification is required.
• RNs appointed or promoted into this position must earn their BSN within four (4) years from the date of hire or promotion.
• Proven clinical nursing experience in chronic disease management.
• Familiarity with terminology and processes associated with chronic diseases.
• Understanding of disease management, including treatment protocols, length of stay considerations, barriers to care delivery, and variations.
• Basic computer proficiency and familiarity with Microsoft Office software.
• Capability to perform necessary visual, auditory, communication, dexterity, computer-use, and patient-care tasks.
• Preferred: Care Management Certification.
• Preferred: Experience in ambulatory transitional care or telephonic triage.
• Preferred: Intermediate computer skills and knowledge of Microsoft Office software.
