Registered Nurse – Diabetes Health Coach Consultant

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Utilize a collaborative approach involving assessment, planning, implementation, and evaluation to engage, educate, and motivate members about their chronic health goals.

• Conduct member assessments utilizing clinical tools and thorough information/data analysis.

• Evaluate members' requirements and benefit-plan eligibility for both integrated internal and external programs and services.

• Assess members' health literacy, technological capabilities, and readiness for change.

• Apply and interpret criteria, guidelines, health/wellness management plans, policies, procedures, and regulatory standards.

• Collaborate with supervisors and other stakeholders to address obstacles to achieving goals and objectives.

• Identify and report quality-of-care issues through the appropriate channels.

• Employ influencing and motivational interviewing techniques to enhance member engagement and encourage lifestyle and behavioral changes.

• Offer coaching, information, and support to enable independent medical and healthy lifestyle choices.

• Interpret and apply clinical guidelines while providing up-to-date healthcare information.

• Assist members in participating knowledgeably in healthcare decision-making with their providers.

• Collaborate with members and/or attending physicians to develop and monitor personalized care plans.

• Adhere to internal policies and procedures in compliance with regulatory and accreditation standards.

⛳️ Requirements

• Must hold a CDCES (Certified Diabetic Educator) certification.

• Active and unrestricted RN licensure in the state of residence is required.

• The state of residence must be a compact state.

• A minimum of 3 years of clinical experience is required.

• Must have or be willing to obtain high-speed broadband internet access.

• Previous coaching experience is preferred.

• Experience in managed care is preferred.

• Proficiency in Microsoft Word, Outlook, Excel, and comfort with various computer programs is preferred.

• Willingness to obtain additional nursing licensure is preferred.

• An Associate's degree is required.

• A BSN is preferred.

• Availability during standard business hours from Monday to Friday, including one evening shift per week starting at 12:30 pm EST.
