Registered Nurse

This is a fully remote position, open to applicants in California, +1 more state.

📋 Description

• Perform comprehensive remote assessments of patients through phone or video conferencing.

• Create, implement, and revise personalized care plans in collaboration with patients and healthcare teams.

• Track patient progress and adherence to treatment plans.

• Offer education and support to encourage self-care and effective disease management.

• Liaise with physicians, specialists, and other healthcare professionals.

• Accurately document patient interactions, clinical observations, and care activities in electronic health records.

• Address patient inquiries and urgent health issues, escalating matters when necessary.

• Engage in quality improvement efforts and pursue ongoing education.

• Maintain clinical skills and enhance patient care services.

⛳️ Requirements

• Valid and unrestricted Registered Nurse (RN) license in the United States.

• License in a compact state, with preference given to those holding additional licenses in California and Illinois.

• At least two years of clinical nursing experience.

• Familiarity with electronic health record (EHR) systems and telehealth technology platforms.

• Excellent communication skills for effectively engaging with patients and interdisciplinary teams remotely.

• Capability to work independently in a remote setting while efficiently managing multiple patient cases.

• Certification in Case Management (CCM) or Telehealth Nursing (CTN) is highly desirable.

• Experience in a remote or virtual care environment is preferred.

• Background in managing complex chronic conditions such as diabetes, heart failure, or COPD is advantageous.

• Knowledge of healthcare regulatory standards and telehealth quality benchmarks is preferred.

• Additional training or education in patient education, motivational interviewing, or behavioral health is preferred.
