This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Identify, develop, and sustain relationships with referral sources including support coordination agencies, educational institutions, and community partners.

• Organize and perform in-person visits, presentations, and follow-ups with partner organizations.

• Participate in and host community events to represent Abound and enhance brand visibility within IDD, Pediatric PDN, and Personal Care markets.

• Generate new client referrals and cultivate warm leads.

• Ensure timely communication and handoff to intake and care coordination teams.

• Monitor referral activity, conversion rates, outcomes, and trends utilizing OnTarget, Microsoft Dynamics, and Power BI.

• Offer continual support to referral partners, including updates on performance, member outcomes, and resolution of any issues.

• Adhere to ethical referral practices, healthcare regulations, and internal policies.

• Report directly to the Chief Growth Officer.

⛳️ Requirements

• Proven experience in business development, outreach, referral relations, or community engagement, ideally within healthcare, human services, or IDD/PDN programs.

• Direct experience in the PA IDD, Pediatric PDN, or Personal Care communities is strongly preferred.

• Exceptional relationship-building, communication, and presentation abilities.

• Competence in analyzing referral data, monitoring KPIs, and reporting on performance trends.

• Familiarity with CRM or case management systems, including Microsoft Dynamics or comparable platforms.

• Highly organized, capable of managing numerous contacts, visits, and deadlines.

• Valid driver's license and readiness to travel locally for community visits and events.

• Bachelor's degree in Business, Marketing, Human Services, Healthcare Administration, or a related field is preferred.
