This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Identify, nurture, and sustain relationships with referral sources like support coordination agencies, educational institutions, and community partners.

• Arrange and conduct face-to-face visits, presentations, and follow-up meetings with partner organizations.

• Participate in and organize community events to represent Abound and enhance brand visibility within the IDD, Pediatric PDN, and Personal Care sectors.

• Create new client referrals and warm leads.

• Ensure timely communication and seamless handoff to intake and care coordination teams.

• Monitor referral activities, conversion rates, outcomes, and trends utilizing OnTarget, Microsoft Dynamics, and Power BI.

• Provide continuous support to referral partners, including updates on performance, member outcomes, and addressing any concerns.

• Adhere to ethical referral practices, healthcare regulations, and internal policies.

• Report directly to the Chief Growth Officer.

⛳️ Requirements

• Experience in business development, outreach, referral relations, or community engagement, preferably in healthcare, human services, or IDD/PDN programs.

• Direct experience within the Pennsylvania IDD, Pediatric PDN, or Personal Care communities is strongly preferred.

• Excellent relationship-building, communication, and presentation abilities.

• Capability to analyze referral data, monitor KPIs, and report on performance trends.

• Familiarity with CRM or case management systems, such as Microsoft Dynamics or similar platforms.

• Highly organized with the capacity to manage multiple contacts, visits, and deadlines.

• Valid driver's license and readiness to travel locally for community visits and events.

• Bachelor's degree in Business, Marketing, Human Services, Healthcare Administration, or a related field is preferred.
