Referral Development Manager

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Expand and enhance referral networks.

• Generate leads for new memberships.

• Assist in converting intakes across the IDD, Pediatric PDN, and Personal Care service lines.

• Identify, nurture, and maintain relationships with support coordination agencies, educational institutions, and community partners.

• Arrange and perform in-person visits, presentations, and follow-ups with partner organizations.

• Participate in and host community events to represent Abound and promote brand visibility.

• Generate new client referrals and warm leads.

• Ensure timely communication and handoff to intake and care coordination teams.

• Monitor referral activity, conversion rates, outcomes, and trends using OnTarget, Microsoft Dynamics, and Power BI.

• Provide ongoing support to referral partners, including performance updates, member outcome reports, and resolution of concerns.

• Ensure adherence to ethical referral practices, healthcare regulations, and internal policies.

• Report to the Chief Growth Officer.

⛳️ Requirements

• Experience in business development, outreach, referral relations, or community engagement, ideally within the healthcare, human services, or IDD/PDN sectors.

• Direct experience in the PA IDD, Pediatric PDN, or Personal Care communities is strongly preferred.

• Excellent relationship-building, communication, and presentation abilities.

• Capability to analyze referral data, monitor KPIs, and report on performance trends.

• Familiarity with CRM or case management platforms, including Microsoft Dynamics or similar systems.

• Highly organized with the capacity to manage multiple contacts, visits, and deadlines.

• Valid driver's license and readiness to travel locally for community visits and events.

• Bachelor's degree in Business, Marketing, Human Services, Healthcare Administration, or a related field is preferred.
