Referral Development Manager

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Enhance and fortify referral networks

• Create new member leads and assist in the conversion of intakes across IDD, Pediatric PDN, and Personal Care service lines

• Act as the representative of Abound Health within the community

• Identify, develop, and sustain relationships with referral sources, including support coordination agencies, schools, and community partners

• Organize and execute in-person visits, presentations, and follow-ups with partner organizations

• Participate in and host community events to represent Abound and enhance brand visibility

• Generate new client referrals and warm leads

• Ensure timely communication and transition to intake and care coordination teams

• Monitor referral activity, conversion rates, outcomes, and trends using OnTarget, Microsoft Dynamics, and Power BI

• Provide continuous support to referral partners, including updates on performance, member outcomes, and resolution of issues

• Ensure adherence to ethical referral practices, healthcare regulations, and internal policies

• Report directly to the Chief Growth Officer

⛳️ Requirements

• Proven experience in business development, outreach, referral relations, or community engagement, ideally within healthcare, human services, or IDD/PDN programs

• Direct experience in the PA IDD, Pediatric PDN, or Personal Care communities is strongly preferred

• Exceptional relationship-building, communication, and presentation abilities

• Capacity to analyze referral data, monitor KPIs, and report on performance trends

• Familiarity with CRM or case management systems, such as Microsoft Dynamics or equivalent

• Highly organized with the capability to manage multiple contacts, visits, and deadlines

• Valid driver’s license and readiness to travel locally for community visits and events

• Bachelor’s degree in Business, Marketing, Human Services, Healthcare Administration, or a related field is preferred
