Referral Development Manager

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Enhance and fortify referral networks

• Create new member leads and assist with intake conversions across IDD, Pediatric PDN, and Personal Care service segments

• Act as the representative of Abound Health within the community

• Identify, nurture, and sustain relationships with support coordination agencies, educational institutions, and community partners

• Arrange and conduct face-to-face visits, presentations, and follow-ups with partner organizations

• Participate in and organize community events to represent Abound and elevate brand visibility

• Generate new client referrals and warm leads, ensuring timely communication and transfer to intake and care coordination teams

• Monitor referral activity, conversion rates, outcomes, and trends using OnTarget, Microsoft Dynamics, and PowerBI

• Provide continual support to referral partners, including performance updates, member outcome information, and resolution of issues

• Ensure adherence to ethical referral practices, healthcare regulations, and internal policies

• Report directly to the Chief Growth Officer

⛳️ Requirements

• Experience in business development, outreach, referral relations, or community engagement, preferably within healthcare, human services, or IDD/PDN programs

• Direct experience in the PA IDD, Pediatric PDN, or Personal Care communities is strongly preferred

• Excellent relationship-building, communication, and presentation abilities

• Capability to analyze referral data, monitor KPIs, and report on performance trends

• Knowledge of CRM or case management systems (such as Microsoft Dynamics or similar)

• Highly organized with the capacity to manage multiple contacts, visits, and deadlines

• Valid driver's license and readiness to travel locally for community visits and events

• Bachelor's degree in Business, Marketing, Human Services, Healthcare Administration, or a related field is preferred
