Referral Development Manager

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Enhance and fortify referral networks.

• Generate new member leads and assist in intake conversions across IDD, Pediatric PDN, and Personal Care service lines.

• Identify, nurture, and sustain relationships with support coordination agencies, educational institutions, and community partners.

• Schedule and perform in-person visits, presentations, and follow-ups with partner organizations.

• Attend and host community events to represent Abound and elevate brand visibility.

• Generate new client referrals and warm leads, ensuring timely communication and transitions to intake and care coordination teams.

• Monitor referral activities, conversion rates, outcomes, and trends using OnTarget, Microsoft Dynamics, and PowerBI.

• Offer continuous support to referral partners, including updates on performance, member outcomes, and addressing any concerns.

• Ensure adherence to ethical referral practices, healthcare regulations, and internal policies.

• Report directly to the Chief Growth Officer.

⛳️ Requirements

• Experience in business development, outreach, referral relations, or community engagement, ideally in healthcare, human services, or IDD/PDN programs.

• Direct experience in the Pennsylvania IDD, Pediatric PDN, or Personal Care communities is strongly preferred.

• Excellent relationship-building, communication, and presentation skills.

• Capability to analyze referral data, monitor KPIs, and report on performance trends.

• Familiarity with CRM or case management platforms, such as Microsoft Dynamics or similar.

• Highly organized with the capacity to manage multiple contacts, visits, and deadlines effectively.

• Valid driver's license and readiness to travel locally for community visits and events.

• Bachelor's degree in Business, Marketing, Human Services, Healthcare Administration, or a related field is preferred.
