Referral Development Manager

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Enhance and bolster referral networks

• Generate new member leads and assist in converting intake across IDD, Pediatric PDN, and Personal Care service areas

• Identify, develop, and maintain relationships with support coordination agencies, educational institutions, and community collaborators

• Organize and carry out in-person visits, presentations, and follow-ups with partner organizations

• Participate in and host community events to represent Abound and enhance brand visibility

• Generate new client referrals and cultivate warm leads

• Communicate and transfer referrals promptly to the intake and care coordination teams

• Monitor referral activity, conversion rates, outcomes, and trends utilizing OnTarget, Microsoft Dynamics, and Power BI

• Provide continuous support to referral partners, including performance updates, member outcome reports, and resolution of concerns

• Ensure adherence to ethical referral practices, healthcare regulations, and internal policies

• Report directly to the Chief Growth Officer

⛳️ Requirements

• Experience in business development, outreach, referral relations, or community engagement, ideally within healthcare, human services, or IDD/PDN programs

• Direct experience in the Pennsylvania IDD, Pediatric PDN, or Personal Care sectors is strongly preferred

• Excellent relationship-building, communication, and presentation abilities

• Capability to analyze referral data, track KPIs, and report on performance trends

• Familiarity with CRM or case management systems, such as Microsoft Dynamics or equivalent

• Highly organized with the capacity to manage multiple contacts, visits, and deadlines

• Valid driver's license required

• Willingness to travel locally for community visits and events

• Bachelor's degree in Business, Marketing, Human Services, Healthcare Administration, or a related discipline is preferred
