This is a fully remote position, open to applicants in North Carolina.
• Oversee and coordinate care transition initiatives for individuals on the High Priority Re-entry list.
• Create and execute transition plans tailored to individual needs and requirements.
• Collaborate with service providers and partners to ensure alignment of services during transition phases.
• Monitor the transition process and take action to resolve any delays or obstacles.
• Facilitate connections to services and guarantee continuity following transitions.
• Assist justice-involved individuals in preparing for their release from the Department of Corrections or county detention facilities.
• Deliver care coordination and transition planning services.
• Synchronize services across various providers and community partners.
• Tackle barriers to promote successful and stable transitions.
• Must fulfill the Qualified Professional (QP) criteria as per North Carolina regulations.
• A minimum of one (1) year of experience with the forensic population, specifically working with individuals in the criminal justice system.
• One qualifying pathway includes: being fully licensed by the NC governing board of Human Services professions; holding a fully licensed RN credential to practice in North Carolina with four (4) years of full-time MH/DD/SA experience; possessing a master's degree in a human service field with one year of full-time MH/DD/SA experience; having a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; or a bachelor's degree in an unrelated field with four years of full-time MH/DD/SA experience.
• A valid driver’s license is required.
• Candidates must reside in North Carolina and be within 75 miles of Trillium’s North Central Region.
• The North Central Region encompasses Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, and Wilson counties.
• Ability to travel within the designated catchment area as needed.
• All applicants must pass a drug test as a prerequisite for employment.
• Preferred: Degree in Human Services or Criminal Justice.
• Preferred: Education, training, and work experience that demonstrate the capacity to work with diverse individuals.
• Preferred: Experience with justice-involved populations or reentry processes.
