PRN Family Nurse Practitioner – Telehealth

This is a fully remote position, open to applicants in Iowa.

📋 Description

• Conduct annual risk assessments in residential environments or through telehealth services within the Nurse Practitioner scope of practice.

• Execute thorough in-home health risk evaluations to identify both active and chronic health conditions along with physical, mental, and social requirements.

• Gather patient histories, carry out examinations, and establish diagnoses.

• Provide written documentation of patient visits in accordance with NCQA standards.

• Measure patient vital signs as needed.

• Make case management referrals and communicate with primary care providers as necessary.

• Engage with patients, caregivers, agency nurses, other healthcare providers, and vendors to ensure accurate diagnosis.

• Perform clinical responsibilities while adhering to OSHA Universal Precautions.

• Safeguard patient confidentiality.

• Attend mandatory meetings and in-services and take part in committees when requested.

• Involve yourself in professional development initiatives and maintain relevant professional licenses and affiliations.

⛳️ Requirements

• Valid and unrestricted nurse practitioner license to practice in the designated states.

• Board certification from ANCC, AANP, or NCCPA.

• Current Basic Life Support (BLS) certification.

• Active enrollment in Medicare and Medicaid programs.

• Must possess a valid driver’s license and maintain a good driving record.

• Exceptional skills in electronic health records (EHR).

• Geriatric training or experience is preferred.

• Ability to work collaboratively and maintain effective relationships with patients, medical personnel, and the public.

• Flexibility to adapt to varying weather conditions and patients’ home settings.

• Capability for a full range of motion including handling and lifting patients.

• Regularly lift and carry items weighing up to 50 pounds.

• Willingness to travel in an automobile or van for up to 150 miles daily in both urban and rural settings.

• Ability to drive, if required.
