PRN Behavioral Health Registered Nurse – Home Care

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Evaluate the physical, behavioral, psychosocial, and environmental requirements of patients.

• Design, coordinate, implement, and assess personalized care plans.

• Manage a designated caseload while performing initial and ongoing nursing evaluations.

• Observe patients for any changes in condition, safety issues, and treatment barriers.

• Relay updates to physicians, clinical leadership, caregivers, and other members of the care team.

• Provide education to patients and caregivers regarding medications, symptom management, safety, and treatment objectives.

• Revise care plans according to the evolving needs and progress of patients.

• Collaborate with an interdisciplinary team to ensure favorable patient outcomes.

• Oversee and document care delivered by LPNs and behavioral health aides when applicable.

• Maintain precise and timely clinical documentation.

⛳️ Requirements

• Valid, unrestricted RN license in the state of practice.

• Associate or bachelor's degree in nursing.

• Minimum of one year of clinical nursing experience.

• Preferred experience in behavioral health, home health, or hospice settings.

• Experience with Homecare HomeBase is preferred.

• Reliable vehicle, valid driver's license, and current automobile insurance are required.

• Willingness to travel within the assigned territory.

• Ability to travel up to 50% of the time.

• Capability to sit, stand, bend, lift, and move intermittently.

• Ability to lift between 50–100 lbs.
