Primary Care Registered Nurse, RN

This is a fully remote position, open to applicants in New Mexico.

📋 Description

• Identify patients with high acuity and risk who could greatly benefit from coordinated care.

• Create interdisciplinary care plans in collaboration with healthcare providers and team members.

• Educate and orient patients, as well as their family/support members, about the care coordination process.

• Coordinate care information and requirements with other healthcare providers.

• Address and resolve issues that may impact the progression of care, while promoting peer support and education.

• Assess adherence to and the efficacy of care plans, making modifications as necessary.

• Facilitate appointments, scheduling, testing, procedures, and remove obstacles to care.

• Provide clinical care in accordance with SCC policy and the New Mexico Nurse Practice Act when required.

• Refer patients to additional providers or resources to address specialized needs.

• Ensure accurate and timely documentation in the EMR.

• Collect and analyze data to support clinical best practices and quality initiatives.

• Assist with research studies related to care protocols, planning, and interventions.

• Engage in staff meetings, conferences, and seminars.

• Perform other relevant duties as assigned.

⛳️ Requirements

• A minimum of 1 year of RN experience in an ambulatory or primary care setting is preferred.

• LPN or Medical Assisting experience may be accepted in place of the RN experience requirement.

• An associate degree in nursing is required.

• A valid Registered Nurse License in New Mexico or another compact state is necessary.

• Candidates must reside in the Albuquerque or Santa Fe, New Mexico local market area.

• Availability to work a standard Monday to Friday schedule from 8 AM to 5 PM.

• Ability to commit to 40 scheduled hours per week.
