Home Care & Hospice Virtual Care Center Coordinator, RN

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Perform virtual evaluations and oversee patients' health remotely.

• Create and coordinate personalized care plans in collaboration with the interdisciplinary team.

• Offer virtual education and support to patients, families, and caregivers concerning symptom management and end-of-life care.

• Collaborate with field nurses, physicians, social workers, and other team members.

• Quickly respond to urgent virtual consultations and assess patient needs.

• Utilize telehealth technology for routine check-ins and follow-up appointments.

• Provide telehealth hospice care and real-time assistance to field staff and caregivers.

⛳️ Requirements

• More than 5 years of experience in Home Care, Hospice, ER, and/or Triage as an RN.

• Current RN license in a Compact State (eNLC).

• Bachelor of Science in Nursing (BSN) from an accredited nursing education program.

• Experience with Epic or Home Care Electronic Medical Records (EMR).

• Excellent communication and interpersonal skills to establish rapport with patients and families virtually.

• Capability to work autonomously as well as part of a collaborative remote team.

• Adaptable to rapid changes in order to meet business or department requirements.

• Experience in patient/customer service.

• Availability for the specified full-time, part-time, or contingent/PRN schedules as required.
