This is a fully remote position, open to applicants in United States.
• Assess and analyze individual practices within designated networks to determine alignment with Guidehealth's value-based care services.
• Initiate outreach and conduct introductory meetings with practice leadership to present Guidehealth's services and value proposition.
• Develop a compelling case for engagement, address any objections, and secure consent to proceed with implementation.
• Collaborate with the Practice Operations Manager to coordinate the kickoff and onboarding for engaged practices.
• Act as the primary point of contact on a daily basis for a designated portfolio of practices.
• Ensure ongoing communication and maintain a regular touchpoint schedule with practice administrators and clinical staff.
• Aid in the long-term retention and satisfaction of practices.
• Communicate practice feedback, requirements, and risks internally, routing or escalating issues as necessary.
• Record practice meetings, key communications, and interactions in HubSpot CRM.
• Organize and coordinate performance reviews, escalation discussions, and unscheduled check-ins.
• Determine suitable meeting frequency and format based on relationship stage, performance trends, and practice preferences.
• Provide monthly performance reviews and facilitate discussions with practice leadership.
• Convert performance data and benchmarks into actionable next steps.
• Assist practices in formulating and tracking improvement plans aligned with performance objectives.
• Identify underperforming practices and escalate relevant context to the Director.
• Collaborate with Guidehealth's clinical pillars: CDI, Care Management, and Quality.
• Convey practice feedback, updates, and operational conditions to internal clinical leadership.
• Present Guidehealth's services and expectations during the implementation kickoff and onboarding process.
• Address internal obstacles or delays that impact practice service delivery.
• Work together with the network's Client Relationship Manager when practices are unresponsive or when additional support is required.
• Assist the Director of Practice Success in preparing network- or client-level presentations and business reviews.
• Contribute to portfolio-level performance narratives and provide context at the practice level.
• Keep the Director updated on network trends, risks, and overall practice sentiment.
• Willingness to travel locally up to 50% of the time.
• Must reside in Georgia.
• A minimum of 3 years of experience in a practice-facing, account management, or business development role within healthcare, value-based care, or a related sector.
• Experience in an ACO, MSSP, or value-based care model, including delegated services, managed care, or population health settings.
• Ability to build trust-based relationships and gain acceptance from physicians and practice leadership.
• Capable of facilitating productive, sometimes challenging, performance discussions with practice leadership.
• Strong comprehension of individual primary care physician practice workflows, daily operations, and front-office/clinical environments.
• Proficiency in managing multiple practice relationships concurrently with a keen attention to follow-through.
• Excellent written and verbal communication skills, with the capability to translate performance data into a coherent narrative for practice audiences.
• Strong organizational and task management skills.
• Judicious in calibrating engagement intensity and format across practices with varying needs.
• Ability to work autonomously with strong ownership and accountability for practice-level outcomes.
• Located in or willing to relocate to the Atlanta, GA area.
• Capability to travel up to 50% of the time within Georgia for in-person practice engagement.
• Preferred experience in practice acquisition, physician outreach, or provider network development.
• A clinical background, such as Medical Assistant, Care Manager, RN, or LPN, is a plus.
• Preferred experience in a managerial or operations role within a primary care practice.
• Health coach certification or skills in motivational interviewing are advantageous.
• Familiarity with CRM platforms, ideally HubSpot.
• Knowledge of quality metrics, HCC coding, or care management integration is beneficial.
• Familiarity with the independent physician practice landscape in the Atlanta area.
• Ability to work during specified hours and maintain a camera-on presence for virtual meetings.
• An internet connection with a minimum of 100 Mbps download and 10 Mbps upload speed is required.
