This is a fully remote position, open to applicants in North Carolina.
β’ Facilitate care coordination, encompassing treatment planning, assessment, monitoring, education, and referrals/linkages.
β’ Assist individuals identified within the Transitions to Community Living Initiative Special Needs Populations.
β’ Oversee care planning and discharge/transition strategies from Adult Care Homes, inpatient units, and detention facilities.
β’ Collaborate effectively with licensed Clinicians, Registered Nurses, and Peer Support Specialists.
β’ Deliver care coordination/care management to designated TCL individuals with behavioral health, physical health, co-occurring, co-morbid, or multi-morbid needs.
β’ Evaluate members transitioning into supportive housing and ascertain additional support necessary for sustained independent living.
β’ Conduct assessments as required.
β’ Record planning and coordination activities accurately and promptly across various software platforms and State database systems.
β’ Must achieve QP status through one of the specified licensure, nursing, or education-and-experience pathways.
β’ Fully licensed by the NC governing board that oversees Human Services professions; or fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing.
β’ RN pathway necessitates four years of full-time experience with the MH/DD/SA population.
β’ A Master's degree in a human service field plus one year of full-time MH/DD/SA experience, or one year of supervised alcoholism and drug abuse counseling experience.
β’ A Bachelor's degree in a human service field plus two years of full-time MH/DD/SA experience, or two years of supervised alcoholism and drug abuse counseling experience.
β’ A Bachelor's degree in a different field plus four years of full-time MH/DD/SA experience, or four years of supervised alcoholism and drug abuse counseling experience.
β’ Must hold a valid driver's license.
β’ Residency in Trillium's Mid State Region: Anson, Guilford, Montgomery, Randolph, or Richmond counties is required.
β’ Ability to travel within the catchment area as necessary.
β’ Must successfully pass a drug test as a condition of employment.
β’ Preferred: Experience and understanding of peer support, and experience working with individuals who are certified peer support specialists.
