Population Health Nurse

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver clinical care management and support for population health to designated members

• Emphasize prevention, wellness, and sustained health outcomes

• Utilize nursing expertise as a licensed registered nurse to evaluate data and determine care necessities

• Organize services employing population health management strategies that enhance outcomes and minimize dependence on episodic care

• Perform clinical evaluations to identify the health needs, risks, and care deficiencies of members

• Create and execute care plans aimed at preventive care, chronic condition management, and wellness objectives

• Facilitate services among providers, care teams, and community resources

• Track member progress and modify care plans based on clinical indicators and results

• Offer health education and coaching to encourage self-management and engagement

⛳️ Requirements

• Associate’s degree in Nursing and a minimum of one (1) year of experience as a Registered Nurse

• Active North Carolina Licensure as a Registered Nurse (RN)

• Current valid driver’s license

• Must be a resident of North Carolina

• Willingness to travel within the designated area as needed

• All applicants must successfully pass a drug screening as a prerequisite for employment
