Physician Assistant (PA-C)/Nurse Practitioner (NP)

This is a fully remote position, open to applicants in California.

📋 Description

• Perform clinical and health-risk evaluations for patients suffering from chronic and intricate conditions.

• Create, oversee, and revise personalized care plans.

• Track patient progress and remote patient monitoring (RPM) data, identifying issues that need intervention.

• Offer education, coaching, and support to patients regarding chronic disease management and medication adherence.

• Collaborate with physicians, pharmacists, nurses, social workers, and other healthcare providers to coordinate care.

• Raise clinical concerns and ensure proper follow-up as necessary.

• Maintain timely, precise, and compliant electronic health record (EHR) documentation.

• Contribute to quality, compliance, and patient engagement initiatives in Chronic Care Management (CCM) and RPM.

⛳️ Requirements

• Valid and unrestricted California PA-C or NP license is mandatory.

• Current national certification (NCCPA, AANP, or ANCC, as appropriate).

• A minimum of 2 years of clinical experience in primary care, internal medicine, geriatrics, chronic disease management, or a similar setting.

• Excellent clinical judgment, communication, and patient care abilities.

• Proficiency in electronic health records (EHR).

• Capability to work independently in a remote setting.

• Access to reliable internet.

• A private workspace that complies with HIPAA regulations.

• Preferred: Experience in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), telehealth, population health, or value-based care.

• Preferred: Experience in managing patients with multiple chronic conditions.

• Preferred: Familiarity with digital health and remote monitoring platforms.
