Physical Therapist – Home Health

This is a fully remote position, open to applicants in California.

📋 Description

• Assess patients’ functional capabilities and create tailored care plans.

• Evaluate home settings and suggest necessary modifications.

• Deliver direct physical therapy treatments to patients.

• Inform patients and their caregivers about therapeutic techniques and the use of equipment.

• Track patient progress and modify treatment plans as needed.

• Work in partnership with physicians and interdisciplinary teams.

• Maintain comprehensive clinical documentation and progress reports.

• Engage in discharge planning processes.

• Oversee Physical Therapy Assistants in accordance with policies and regulations.

• Provide in-home therapy services following a physician’s directives.

• Assist patients in regaining their mobility and achieving independence.

⛳️ Requirements

• Valid California Physical Therapy License.

• At least 2 years of experience in physical therapy (home health experience preferred).

• Excellent communication and organizational skills.

• Current CPR certification.

• Possession of a valid driver’s license and access to a reliable, insured vehicle.
