This is a fully remote position, open to applicants in California.
• Assess patients’ functional abilities, mobility, muscle strength, gait, and range of motion to create personalized treatment plans.
• Provide physical therapy services in alignment with the physician’s care plan and the California Physical Therapy Practice Act.
• Instruct patients and their families or caregivers on therapeutic exercises, equipment usage, and recovery objectives.
• Work in collaboration with physicians and the care team by reporting any changes in patient condition and engaging in case conferences.
• Oversee Physical Therapy Assistants and, when physical therapy is the only skilled service, supervise Home Health Aide care bi-weekly.
• Handle documentation, including clinical progress notes and discharge planning.
• Recognize the need for additional home health services and facilitate referrals as necessary.
• Carry out other responsibilities as required for patient care.
• Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.
• Valid California Physical Therapy license.
• At least 2 years of experience in physical therapy; home health experience is advantageous.
• Excellent communication skills with the ability to engage with patients and their families.
• Current CPR certification from the American Heart Association.
• Valid driver’s license and a reliable, insured vehicle.
