This is a fully remote position, open to applicants in California.
• Assess patients’ functional abilities, mobility, muscle strength, gait, and range of motion to create tailored treatment plans.
• Provide physical therapy services aligned with the physician’s care plan and adhere to California’s Physical Therapy Practice Act.
• Inform patients and their families or caregivers about therapeutic exercises, equipment usage, and recovery objectives.
• Collaborate with physicians and the healthcare team, noting changes in patient conditions and engaging in case conferences.
• Supervise Physical Therapy Assistants and, in instances where PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation, including clinical progress reports and discharge planning.
• Recognize the need for additional home health services and coordinate referrals as necessary.
• Execute other responsibilities as required for patient care.
• Bachelor’s or Master’s degree in Physical Therapy from a recognized institution.
• Current California Physical Therapy license.
• At least 2 years of physical therapy experience; experience in home health is advantageous.
• Excellent communication skills and the ability to engage with patients and their families effectively.
• Valid CPR certification from the American Heart Association.
• Possession of a valid driver’s license and access to a reliable, insured vehicle.
