This is a fully remote position, open to applicants in California.
• Assess patients’ functional abilities, mobility, muscle strength, gait, and range of motion to create personalized treatment plans.
• Provide physical therapy services in alignment with the physician’s care plan and adhere to California’s Physical Therapy Practice Act.
• Inform patients and their families or caregivers about therapeutic exercises, equipment usage, and recovery objectives.
• Collaborate with physicians and the healthcare team, communicate changes in patient status, and engage in case conferences.
• Supervise Physical Therapy Assistants and, when Physical Therapy is the only skilled service, oversee Home Health Aide care on a bi-weekly basis.
• Manage documentation, including clinical progress reports and discharge planning.
• Identify the need for additional home health services and facilitate referrals as necessary.
• Perform other duties as required to support patient services.
• Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.
• Valid California Physical Therapy license.
• At least 2 years of experience in physical therapy; experience in home health is advantageous.
• Excellent communication skills with the ability to engage with patients and their families.
• Current CPR certification from the American Heart Association.
• Valid driver’s license and a reliable, insured vehicle.
