This is a fully remote position, open to applicants in Illinois.
• Advocate for Elara Caring’s philosophy, mission statement, and administrative policies to guarantee quality care.
• Deliver direct physical therapy services in alignment with accepted practice standards and the patient physician’s certification.
• Adhere to the Plan of Care established by the Physical Therapist under supervision.
• Promptly report any changes in patient status to the Physical Therapist.
• Confirm the Plan of Care prior to each visit.
• Administer care per physician’s orders, assessment data, and established standards and guidelines.
• Integrate patient care objectives from the Plan of Care into therapy sessions.
• Educate patients and families on the safe usage of prosthetic, orthotic, and assistive devices.
• Guide patients or caregivers in the therapy home program.
• Provide physical therapy assistant services via home visits.
• Report to the Branch Director, Clinical Manager, Clinical Supervisor, or designated individual.
• Graduate from an accredited physical therapy assistant program recognized by the American Therapy Association.
• Hold a current, unrestricted PTA license valid in the state of employment.
• Preferably possess over 6 months of experience as a Physical Therapist Assistant.
• Willingness and ability to travel within the branch/office coverage area.
• Reliable vehicle required.
• Valid driver’s license necessary.
• Current auto insurance compliant with state regulations.
• Capable of and willing to travel 50% of the time.
• Able to sit, stand, bend, lift, and move intermittently.
• Capable of lifting between 50–100 lbs.
• Physically fit to work in a high-stress environment.
• Willing to work irregular hours, including on-call hours if applicable.
• Commitment to adhere to State Practice Act Guidelines.
