This is a fully remote position, open to applicants in New York.
β’ Approve the ordering of medications for patients receiving treatment at the medical center.
β’ Supervise and actively engage in the compounding, preparation, and dispensing of medications in compliance with state and federal pharmacy laws and regulations.
β’ Manage pharmacy technicians and guide their tasks to ensure efficient workflow.
β’ Adhere to dispensing policies and procedures; dispense medications accurately and in a timely manner.
β’ Independently address and resolve medication order issues.
β’ Process and verify medication orders utilizing current information system technology; create and maintain precise patient profiles.
β’ Review products that require pharmacist approval prior to dispensing.
β’ Engage in quality assurance activities, medication incident reporting and follow-up, MAR reconciliation, and reporting of medication adverse events/errors.
β’ Collaborate or participate in clinical practice with physicians, nurses, healthcare professionals, and patients as part of an interdisciplinary team.
β’ Take part in targeted clinical initiatives and Medication Use Evaluations.
β’ Monitor designated patient populations for clinical outcomes and suggest therapy adjustments as necessary.
β’ Document clinical activities and interventions effectively.
β’ Educate pharmacy staff and healthcare professionals; provide patient education when required.
β’ Address patient-care and operational challenges through effective communication and reasonable efforts at resolution.
β’ Undertake charge pharmacist responsibilities, including coordinating shift resources, resolving problems, mediating staff conflicts, and ensuring clear shift-to-shift communication.
β’ Notify supervisors when issues exceed personal expertise or authority.
β’ Carry out additional duties as assigned.
β’ A Bachelor's degree in Pharmacy is required.
β’ A minimum of 1 year of relevant experience in acute care pharmacy or successful completion of an accredited pharmacy practice residency is required.
β’ Pharm D is preferred.
β’ An equivalent combination of education and experience will be considered.
β’ A valid Pharmacist license is required upon hire.
β’ Must be able to adhere to pharmacy, departmental, and hospital policies and procedures.
β’ Proficiency in accurately and promptly dispensing medications is essential.
β’ Capability to resolve medication order problems is necessary.
β’ Experience in supervising pharmacy technicians is required.
β’ Ability to engage in clinical practice, medication-use evaluations, quality assurance, and interdisciplinary patient care is important.
β’ Competency in monitoring renal/hepatic function, culture results, and other relevant laboratory tests is required.
β’ Must be able to educate pharmacy staff, healthcare professionals, and patients effectively.
β’ Ability to perform charge pharmacist duties when scheduled is required.
