This is a fully remote position, open to applicants in United States.
β’ Facilitate patient care activities and serve as a communication link between patients, providers, and clinical personnel.
β’ Organize and oversee patient appointments, follow-up visits, and referrals.
β’ Ensure the accuracy and currency of patient records within electronic health record (EHR) systems.
β’ Track patient progress and proactively identify and resolve care gaps or scheduling issues.
β’ Offer patient education and guidance related to care plans and subsequent steps.
β’ Work collaboratively with clinical teams to guarantee adherence to care protocols and regulatory requirements.
β’ Address patient inquiries, concerns, and escalations with professionalism and empathy.
β’ Previous experience in patient coordination, clinical coordination, or a healthcare administrative role.
β’ Knowledge of EHR/EMR systems and healthcare management software.
β’ Strong understanding of medical terminology and clinical processes.
β’ Exceptional communication and interpersonal abilities with a focus on patient satisfaction.
β’ Capability to multitask, prioritize responsibilities, and work independently in a remote setting.
β’ High school diploma or equivalent is required.
β’ Dependable internet connection is essential.
β’ A dedicated, distraction-free home workspace is necessary.
β’ Willingness to commit to a 40-hour workweek with rotating shifts from Monday to Sunday.
