Patient Engagement Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Take ownership of the patient conversion funnel and ensure that referrals are contacted within established SLAs for prompt access to care.

• Lead outreach efforts via phone and text to new patient referrals, introducing the program, explaining its functionality, and addressing any inquiries.

• Address patient objections and concerns with empathy.

• Assist patients through the informed consent process and ensure that documentation is accurate.

• Schedule intake sessions, confirm attendance, and manage reminders as well as rescheduling.

• Work through recurring lists of cancellations, no-shows, and program drop-offs to re-engage patients.

• Re-educate and re-motivate disengaged patients, facilitating rescheduling and re-consenting as necessary.

• Monitor outreach activities and outcomes in CRM/EHR systems.

• Report on the time taken from referral to outreach, outreach to scheduled intake, conversion rates, and completed intakes.

• Collaborate with Clinical and Operations teams to hand off scheduled patients and identify potential improvements in processes or messaging.

⛳️ Requirements

• Experience in phone-based customer service, call center roles, sales development, or patient-facing healthcare coordination.

• Ability to meet conversion and pipeline targets comfortably.

• Excellent communication skills for educating patients, building trust, and addressing objections.

• Highly organized and detail-oriented; capable of simultaneously managing an active referral pipeline and a re-engagement queue.

• Display empathy and patience when dealing with confused, frustrated, or hesitant patients.

• Proficient in using CRM/EHR systems and accurately documenting consent.

• Experience in healthcare, medical sales, or patient enrollment is advantageous.

• Spanish bilingual skills are a plus.

• Required application experience questions should cover phone-based/customer service or similar experience, performance targets, outbound patient/customer calls, CRM/EHR pipeline or caseload management, and Spanish communication.
