Patient Care Navigator

This is a fully remote position, open to applicants in Arizona, +8 more states.

📋 Description

• Assist in managing daily incoming calls, fax referrals, emails, and departmental responsibilities

• Support Care Managers with their designated tasks

• Regularly assess and evaluate patient care to ensure progress towards targeted outcomes

• Identify and address obstacles that impede effective patient care

• Enhance the quality and thoroughness of documentation

• Collaborate with patients, families, nurses, physicians, and other team members

• Build consensus on issues and resolve them in a timely and appropriate manner

• Maintain clear, complete, concise, and organized documentation

• Monitor clinically low-risk cases

• Inform the Care Manager and/or Medical Director of any new or concerning clinical issues

• Implement necessary actions to ensure quality and appropriate care

• Accept referrals and identify patients who meet care management criteria

• Support team members in upholding HOPCo’s values and mission

• Assist with the planning, development, and evaluation of programs

• Perform related and other assigned duties within the CIN


⛳️ Requirements

• Preferred Medical Assistant or Certified Nursing Assistant degree, or equivalent experience

• At least three (3) years of experience as a certified nursing assistant or medical assistant is preferred

• One year of experience in a medical business office, physician’s office, or hospital setting is preferred

• Experience in value-based care or population health is preferred

• Experience in orthopedics or musculoskeletal specialties is preferred

• Knowledgeable in the health care field and orthopedics

• Familiarity with policies and procedures related to patient care

• Understanding of patient confidentiality, state laws, HIPAA regulations, and medical records procedures

• Knowledge of medical practices, terminology, and reimbursement policies

• Proficiency in Microsoft Office applications

• Ability to follow up with patients and respond to inquiries efficiently

• Professionalism and skill in interactions with providers, patients, family members, and the public

• Ability to understand patient education needs by effectively communicating information to patients and families

• Strong problem-solving and time management skills

• Competence in evaluating the effectiveness of current methods and procedures

• Excellent verbal and written communication skills

• Ability to read, interpret, and apply policies and procedures

• Skill in prioritizing multiple requests

• Ability to interact effectively with patients, medical and administrative staff, and case managers

• Capability to thrive in a fast-paced environment
