Patient Care Navigator

This is a fully remote position, open to applicants in Arizona, +8 more states.

📋 Description

• Supervise daily inbound calls, fax referrals, emails, and departmental tasks.

• Support Care Managers with assigned responsibilities.

• Monitor patient care through assessment and evaluation to ensure progress towards desired outcomes.

• Identify and address obstacles to effective patient care.

• Enhance the quality and completeness of documentation.

• Collaborate with patients, families, nurses, physicians, and other team members.

• Document information clearly, completely, concisely, and in an organized manner.

• Monitor clinically low-risk cases.

• Notify the Care Manager and/or Medical Director of new or concerning clinical issues.

• Implement necessary actions to promote quality and appropriate care.

• Accept referrals and identify patients who meet care-management criteria.

• Uphold HOPCo’s values and mission.

• Assist with program planning, development, and evaluation.

• Perform related duties and other responsibilities as assigned within the CIN.


⛳️ Requirements

• Preferred: Medical Assistant or Certified Nursing Assistant degree or equivalent working knowledge.

• Minimum of three (3) years of experience as a certified nursing assistant or medical assistant is preferred.

• At least one year of experience in a medical business office, physician’s office, or hospital setting is preferred.

• Experience in value-based care or population health is preferred.

• Experience in orthopedics or musculoskeletal specialty is preferred.

• Knowledge of the healthcare field and orthopedics.

• Familiarity with policies and procedures related to patient care.

• Understanding of patient confidentiality, state laws, HIPAA regulations, and medical records procedures.

• Knowledge of medical practices, terminology, and reimbursement policies.

• Proficiency in Microsoft Office applications.

• Skills in following up with patients and responding to inquiries promptly.

• Professionalism and skill in interactions with providers, patients, family members, and the public.

• Ability to understand patient education needs by effectively communicating information to patients and families.

• Proficiency in problem-solving and time management.

• Skills in evaluating the effectiveness of current methods and procedures.

• Strong verbal and written communication skills.

• Ability to read, interpret, and apply policies and procedures.

• Capability to prioritize among multiple requests.

• Ability to interact effectively with patients, medical and administrative staff, and case managers.

• Capacity to work in a fast-paced environment.

• Must read and sign HOPCo Mission, Vision, and Values.
