This is a fully remote position, open to applicants in Arizona.
• Collaborate closely with the SCAN Mobile Care Team clinicians to provide an outstanding healthcare experience for members.
• Refill authorized prescriptions, liaise with vendors, order medical equipment for patients, process laboratory orders, and complete necessary patient documentation.
• Arrange specialty appointments and coordinate transportation services.
• Manage incoming phone calls and direct them appropriately.
• Organize and distribute incoming mail and faxes.
• Maintain the inventory of clinical supplies and order additional supplies as needed.
• Collect and organize patient medical information and reports while ensuring confidentiality and safeguarding patient privacy.
• Assist in patient care initiatives.
• Identify opportunities for improvement based on patient feedback and suggest feasible solutions.
• Occasionally accompany clinicians in communities within Los Angeles and Orange Counties to enhance patient care.
• Monitor and address clinical quality care gaps in collaboration with clinicians.
• Support the SMCT sales team with the setup and teardown of presentations and events at senior-living communities during peak enrollment periods.
• Advocate for SCAN Health Plan’s mission, vision, and values, serving as an ambassador within senior-living communities.
• Actively contribute to and promote SCAN’s vision, mission, and strategic objectives.
• Perform additional duties as assigned.
• Associate's Degree or equivalent experience is required.
• A minimum of 2 years of relevant experience as a medical assistant, medical office administrator, or care coordinator is necessary.
• Direct patient care experience is preferred but not mandatory.
• Familiarity with Medicare Advantage or long-term care communities is preferred.
• Effective verbal and written communication skills are essential.
• Cultural competence and sensitivity are required.
• Multilingual abilities are preferred.
• Must be highly organized with strong attention to detail.
• Strong interpersonal skills, a hospitality mindset, and the ability to be a change agent are crucial for building relationships with the healthcare team, patients, families, and senior-living communities.
• Knowledge of HIPAA regulations and the ability to maintain compliance is necessary.
• Must be adaptable, open to change, and skilled in problem-solving.
