Patient Care Coordinator

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the primary point of contact for patients and their families.

• Assist older adults and caregivers throughout the intake procedure.

• Offer patient support, coordinate care, and provide emotional assistance.

• Aid patients in navigating their entry into care services.

• Pair patients with the appropriate therapist for their needs.

• Ensure that patients feel supported, understood, and confident during the process.

• Manage multiple patients at various stages while maintaining process continuity.

• Take initiative and contribute to the development of key processes.

⛳️ Requirements

• Must be located in Canada.

• Empathetic and effective communicator.

• Highly organized with a keen attention to detail.

• Comfortable with learning Google Workspace, EHR platforms like Healthie, and CRMs such as HubSpot.

• Quick to adapt to new software applications.

• Passionate about enhancing mental healthcare access for older adults.

• Experience in healthcare, customer service, or administrative support is advantageous but not mandatory.

• Willingness to work during Eastern Time Zone hours.

• Must be open to working one of the specified shifts: 9a–6p ET or 12p–9p ET.
